Does anyone have a Hook, Line, & Sinker template?

The format has been used heavily since the Rifts Indexes, and I'm interested in adapting some ideas to that format. An existing template would ease work.

Comment: Years of life and still wanting to add some personal horror to its mix. Can't wait for bio-suits to reign supreme!

Hook: A possible scenario in which the players interact with a problem or odd encounter that has some following lead up. An example would be that someone had jumped off a bridge and below them was a strange orchard growing, it caught the man and now the bridge is being closed off for investigation.

Line: A follow up where someone finds out a possible issue is going to arise from the scenario, or maybe that said scenario is going to become less relevant. An example from the previous one is that someone found out the orchard is actually growing on a large mammal that is slowly swimming out into the bay, someone landed on the brush and ate from the orchard, doctors had to rush them into the ER as they were starting to look less and less human and the guy was screaming in pain.

Sinker: A sinker is simply the conclusion to the story, where everything leads to its final climax. The last example would be that the man from earlier is nowhere to be seen, but out by the bay leading out into the ocean you can see two creatures out there, one with the orchard and one that growing it, both seem content and where the man was hospitalized there was a series of strange splotches, cloths ripped up, and odd foot prints that don't match anything that would fit their profile. Turns out this creature was a mutating symbiote that wanders ocean banks and rivers for phytoplankton and was just following its routine out after being woken up from someone blast mining in the mountains nearby.
