It would be the weapon damage plus the supernatural punch damage plus the normal damage bonus for high P.S., as stated on page 35 of the Nightbane RPG.

So, if you have an Sn PS of 25 and your hand is transformed into a battle axe (or even if you're just wielding a battle axe normally), you'll do 2D4+2D6+10 (14-30) damage with a restrained slash, 5D6+10 (15-40)damage on a full strength slash, and 8D6+10 (18-58) damage on a power slash.
