Going back to the OP I actually based a whole adventure on a Rawhead (CB3 pg. 60). That monster really inspired me!
Just pick a monster and think how it can mess with PC. I usually pick those they aren't just combat oriented (Dar'ota anyone? - CB3 pg. 73). Have fun!
